Health Screening Questionnaire

In order to keep everyone safe, all Members & Guests must complete and pass our Online Health Screening Questionnaire prior to participation in our programs and entering our facilities. 

Steps to Complete Questionnaire:

1. Click on the link below for the club you are visiting 

2. Answer the screening questions

3. Click submit

4. Show your pass result to the Front Desk

What Happens If You Do Not Pass?

If you do not receive a pass result, please do not proceed to reception for check-in.

Mayfair Clubs encourages you to seek medical care and advises you to follow public health guidelines which would be to go home and self isolate.  ​

Please contact the Club Manager and let them know your status to confirm the cancellation of your service bookings if applicable.
